• Intensive care is not included; ICU daily rate is $1,500.
• Patient’s medical condition determines package applicability — patients requiring tracheostomy follow-up or intensive critical care may require a different package.
• Botox (botulinum toxin) medications, nerve blocks, and joint injections are not included.
